Share your views on ‘One Health and Care’ shared care record

Our local shared care record, One Health and Care was launched within across local GP practices in November 2022 and at SWB in November 2023, providing valuable patient information to clinical staff.

The One Health and Care project team would like to gather your insights and feedback following the launch at the Trust so they can better understand how staff are using it and identify any gaps in functionality or data to help improve your experience.

The survey will take around five minutes to complete where we will ask you about your experiences of using the shared care record, as well as capturing any concerns or suggestions you may have the results will play a pivotal role in shaping the future of our local shared care record, enabling the project team to enhance its effectiveness and efficiency.
