Nutrition Teams MDT

The nutrition team MDT will now occur at 11am -12.30pm  on Tuesday mornings via TEAMS. Please refer any patients that need to be discussed to  swbh.acutenutritionnurses@nhs.net. Please could we ask for a proforma (provided by the acute nutrition team) and a member of the team to be present at the MDT.
