Incident investigation training

Are you interested in completing incident investigation training? This training is aimed at specialty and independent leads who will lead on serious incident investigations.

Note: You will be asked to lead on two investigations within the next 12 months.

The training session will include:

  • An understanding of why incidents occur, and what we can learn from them.
  • How events are classed as serious incidents and what this means.
  • What evidence to collect and how to represent this.
  • How we support people involved and affected by the event.
  • Analysing the evidence so that root causes and contributory factors are identified.
  • How to write up the final investigation report.

Training sessions will run from 9.30am – 2pm on the following dates:

  • Wednesday 27 April, clinical skills room, Postgraduate Centre (eight spaces left)
  • Wednesday 18 May, clinical skills room at Postgraduate Centre (nine spaces left)
  • Tuesday 14 June, room 12/13, Sandwell Education Centre (ten spaces left)
  • Wednesday 20 July, clinical skills room, Postgraduate Centre (ten spaces left)
