E-virtual clinic referral form for AMU/SDEC should be accessed through Unity

As you may be aware, up until Monday 21 October, the E-virtual clinic referral form for AMU/SDEC was available on Connect. This form has now been disabled.

Should any colleagues need to do an E-virtual clinic referral for AMU/SDEC, they should now do this through Unity.
