Are you using SWB shared hospital care record (SHCR)?

Staff are encouraged to use SWB shared hospital care record (SHCR) to access patient information from across Birmingham, the Black Country, and the West Midlands. One of the biggest challenges when managing our patients has been our ability to access information on our patients from all the different organisations that we work with. The joining up of data from multiple partner organisations will help us improve the care we provide patients, e.g. reducing duplication of tests already carried out elsewhere; ensuring that multi agency safeguarding hubs have access to a comprehensive view of information from across health and social care; future development include providing access to advanced care plans and resuscitation decisions.

Since Unity was launched in November 2019, we have had access to local GP data including current medications and diagnoses via Cerner’s Health Information Exchange (HIE), and more recently we have been able to see virtual ward entries from SystmOne.

Access to the one health and care shared care record (OHC SCR), a separate digital platform which will provide access to information from across the Black Country (GP records, allergies and hospital appointment data; future expansion includes blood results, radiology, clinical correspondence, care plans) provided by a wide range of health and social care organisations. OHC SCR will be accessed by a link on the left-hand menu in Unity, and a dedicated link in SystmOne.

The first time you access this you need to enter your name and role so that the system can link your profile to the shared care record.
