A more user friendly Supportive Care Plan

The Supportive Care Plan (SCP) Medical Module is now more easily edited and updated, so your team can be confident that you are focusing on whats important to the people we care for as their needs evolve.

This will help you get people to where they want to be in their last months, weeks or days of life including fast-track discharge where appropriate. The nursing modules are unchanged and continue to be the best places to document symptoms, how they being addressed and care support need for our patients and their families.
