CIWA interpretation questions

Alcohol withdrawal syndrome is a common medical problem and follows the abrupt discontinuation of, or at least the rapid decrease in intake of alcohol, the use of which has been heavy and prolonged. The syndrome usually begins within 6-8 hours of alcohol cessation or reduction, though alcohol withdrawal delirium does not usually develop for 2–3 days after stopping drinking and usually lasts for at least 48–72 hours.

If patient admission not avoided follow management plan and refer to Alcohol Withdrawal Guidelines via connect. Based on undertaking CIWA scoring it may be useful to use this scoring interpretation guideline alongside your assessment.

a) Nausea & Vomiting. Ask ‘Do you feel sick?’ ‘Have you vomited?’

b) Paroxysmal sweats. Use observation. Is the patient clammy/sweaty? Beads of sweat to forehead? Drenching sweats?

c) Anxiety. Ask ‘Do you feel nervous?’ ‘Do you feel calm inside or churned up?’ Use observation is the patient at ease?

d) Agitation. Use observation. Is the patient restless/fidgety?

e) Tremor. Use observation. View the patient from afar before approaching them. Is their tremor visible? How severe when patients arms extended. Is the tremor severe even with arms not extended?

f) Headache. Ask patient. How severe?

g) Auditory disturbances. Ask ‘Are you more aware of noise around you?’ ‘Are you hearing things you know are not there?’

h) Visual disturbances. Ask ‘Does the light appear to be too bright?’ ‘Does it hurt your eyes?’ ‘Are you seeing things which you feel may not be real?’

i) Tactile disturbances. Ask ‘Do you have any itching, pins and needles sensations, burning or numbness?’

j) Orientation. Ask ‘What day is it?’ ‘Where are you?’ ‘Who am I?’

Ramadan 2018

Ramadan is the ninth month of the Islamic calendar, and is observed by Muslims worldwide as a month of fasting to commemorate the first revelation of the Quran to the Prophet Muhammad. This annual observance is regarded as one of the Five Pillars of Islam.

During this month the SWBH Muslim Liaison Group who are a part of the BME Staff Network alongside the Trust wanted to take the opportunity to highlight famous and important members of the Islamic Faith.

Today we look at Anser Khan who is a security officer at City Hospital and co-chair of the BME Staff Network.

Anser believes that working in such a diverse and multi-cultural organisation with many, faiths and beliefs, it is imperative that everyone is respected and treated fairly and equally. Religious, cultural and social barriers should be broken down if we are to have harmonious environments at work and in society.

Deprivation of Liberty Safeguards – getting the referral right

Deprivation of Liberty Safeguards is the procedure prescribed in law when it is necessary to deprive of their liberty a resident or patient who lacks capacity to consent to their care and treatment in order them safe from harm.

Unfortunately some referrals are being delayed due to some common mistakes. This means Best Interest Assessors are then delayed in assessing patients, leading to unnecessary delays in care.

The list below shows three common mistakes that colleagues make when submitting their referrals:

  • Sending DoLS referrals to incorrect local authority or not sending to the local authority at all (some are getting sent to the Dementia, Delirium, and Distress email address which is incorrect).
  • Dates on the referrals are incorrect – DoLS can be authorised for 7 days which includes the day you are submitting i.e. if you are submitting on the 12th of the month it will be authorised until the 18th.
  • You can extend this 7 days for a further 7 days which is at the bottom of the DoLS referral form.  You can complete this if you feel the patient will be in hospital for longer than 7 days or if the previous 7 days are running out and a best interest assessor hasn’t been yet.  Again this date is including the submission day so if you are sending on the 12th of the month it will be authorised until the 25th.

For more information, you can access the DoLS help document and the DoLs Q&A document

For further support and guidance in completing DoLS assessments, please contact the Adult Safeguarding Team:
